Managing Dependents on Benefits

Add or remove dependents from your health insurance and other benefits.

Eligible Dependents

• Spouse or domestic partner
• Children under age 26 (biological, adopted, stepchildren)
• Disabled dependents over 26 (with proof)

When You Can Make Changes

1. Open Enrollment (November annually)
2. Within 30 days of:
   • Marriage/domestic partnership
   • Birth or adoption
   • Divorce or legal separation
   • Dependent loss of other coverage
   • Dependent ages out (turns 26)

Adding a Dependent

1. Log into UKG within 30 days of qualifying event
2. Go to Benefits > Change Benefits
3. Select "Qualifying Life Event"
4. Choose event type and enter date
5. Click "Add Dependent"
6. Enter dependent information:
   • Full name
   • Date of birth
   • SSN
   • Relationship
7. Upload required documents
8. Select benefits for dependent
9. Submit

Required Documentation

• Birth: Birth certificate
• Adoption: Adoption decree
• Marriage: Marriage certificate
• Domestic Partner: Affidavit of Domestic Partnership

Removing a Dependent

1. Benefits > Change Benefits > Life Event
2. Select removal reason (divorce, aged out, etc.)
3. Choose dependent to remove
4. Upload documentation if required
5. Submit

Cost Impact

Adding dependents increases premiums:

• Employee only → Employee + Spouse: ~$150/month
• Employee only → Employee + Child(ren): ~$120/month
• Employee only → Family: ~$250/month

Late Enrollment

If you miss the 30-day window:

• Must wait until next open enrollment
• Exception: Loss of other coverage
