Life Insurance Benefits

BetterHealth provides life insurance coverage at no cost to employees.

Basic Life Insurance

• Coverage: 1x annual salary (up to $500,000)
• Cost: Company-paid, no premium deductions
• Beneficiary: You designate who receives benefit
• Automatic: Coverage begins day 1 of employment

Supplemental Life Insurance

Purchase additional coverage at group rates:

• Employee: Up to 5x salary (max $1,000,000)
• Spouse: Up to $250,000
• Child(ren): $10,000 per child
• Cost: Based on age, deducted from paycheck

Designating Beneficiaries

1. Log into UKG
2. Go to Benefits > Life Insurance
3. Click "Beneficiaries"
4. Add primary and contingent beneficiaries
5. Include full name, relationship, % of benefit, SSN
6. Review annually or after major life events

AD&D Insurance

Accidental Death & Dismemberment included:

• Same benefit amount as basic life
• Pays for accidental death or serious injury
• Automatic coverage with basic life insurance

Filing a Claim

In the event of a death:

1. Beneficiary contacts HR: [e-mail]
2. Provide death certificate
3. Complete claim forms (sent by insurance carrier)
4. Claims processed within 30 days
